This is your Awardee Questionnaire for the 2024 FWN Magazine Commemorative Issue. It features all the 2024 Global FWN100™ Awardees, Awardees from 2007-2023 attending the 2024 Filipina Summit, the Summit Schedule, and other articles about Filipina women in the diaspora.

The FWN Magazine is lovingly produced by an all-volunteer editorial and production team. KUDOS to our amazing volunteers helping FWN achieve its mission.

To avoid production delays and ensure error-free publication of your personal information, please have ALL information requested ready and complete this form before 15 OCTOBER.

IMPORTANT DISCLOSURE:

  • What you input on this form will get published, as written, in the FWN Magazine 2024 Global FWN100™ COMMEMORATIVE Issue. Please be mindful of grammar and spelling. Purchase the app, Grammarly, which will help with spelling and syntax (the arrangement of words and phrases to create well-formed sentences).

  • Your information is subject to editorial editing for length and clarity to meet FWN requirements, publication guidelines, and space availability.

  • When you submit this form, you release Filipina Women's Network from all liability. Your information becomes part of FWN's historical archive about Filipina women.

 

Before You Begin

  • SAVE & CONTINUE LATER: You are able to save partial entries after filling in at least one field. So prepare the answers to the nomination questions on a Word document before you attempt to initiate an online nomination.

    • Once you fill out the first field on your form, the PROGRESS BAR and the SAVE icon will appear at the bottom of your form.

    • The PROGRESS BAR will allow you to jump directly to any page in your form.

    • The SAVE icon will allow you to SAVE & CONTINUE LATER. You need to click the SAVE icon in order to save your progress.

    • Make sure you click the NEXT (or PREVIOUS) button after you fill out a field.

    • Simply filling out a field then leaving the form won't save it to our servers.

    • You can either click the GET SHAREABLE LINK to get your SESSION URL or click the SEND THE LINK AS EMAIL button if you want to send it to your email address for later usage. Do not lose your unique link. You will have to start over if you lose your unique link.

  1. You signed and initialed all pages of your Global FWN100™ Terms and Acceptance, and sent it to fwn100@ffwn.org (or replied to your Award Notification Letter).

  2. Joined FWN at the Global FWN100™ level, if you have been approved for the award — If not, join here; or upgrade to the LIFETIME Awardee level at https://filipinawomensnetwork.org/lifetime-members/

  3. Registered for the Full Access Summit Pass. If not, register here.

  4. Reserved your hotel room when you registered for the Filipina Summit

  5. Secured your air travel information. We will need your travel itinerary on this form.

  6. Signed up as a Femtor — http://www.femtormatch.org/femtor-application/

Info we need from 2024 Global FWN100™ awardees and 2007-2023 Awardees Registered for the Filipina Summit in SYDNEY:

  1. Your NAME as you want it engraved on your crystal award - we had to order the award already and we used the name you indicated on your Nomination Form.

  2. Respond to the questionnaire below - this is the FUN part!

  3. Your bio is in 100 words or less. Please honor this requirement. We don't have a lot of space!

  4. IMPORTANT: Your updated professional headshot - digital color photo in JPEG format; 300 DPI resolution. Use a photo editor if necessary. You can find online photo editors at http://bit.ly/photo-editors.

  5. LABEL your photo and bio with your full name: like this - [FIRST_LASTNAME] - for example, MARILY_MONDEJAR - We receive so many photos and we want to make sure that the photo we feature matches your name!

 

If You Wish To Submit An Article For Publication:

  1. Your article has to be an uploadable text document (e.g., Word document only). Label your document with the title of the article and your name. 300 words maximum (honor this word limit, please)

  2. Any digital photos or images (300 DPI resolution, JPEG format) to accompany your article. Please label the photos and input the captions in the space provided. Maximum two photos/images that expand the essence of your article.

  3. Your article should be about the 19th Filipina Global Leadership Summit theme - INTERSECTIONALITY: FILIPINA WOMEN CHANGEMAKERS CREATING A BETTER WORLD. Learn more at www.filipinasummit.org

  4. Please know that we do not guarantee the publication of your article as space is based on advertising support received. If your article does not make the magazine, we will consider featuring it in the ePahayagan. Please help by advertising in the magazine.
